REGISTRATION INFORMATION
INDIVIDUAL: Players are welcome to sign up as an individual and will be placed on a team. The cost of the individual player registration is $150 + HST ($169.50) per session.
TEAM: Players may enter as a team with a completed team registration form, roster and individual player registration forms. Team registration is $1575 + HST ($1779.75) per session.
INSURANCE: In addition to the registration fees, all players must pay a $20 OSA fee for insurance. This will cover players until May 2012. All registration and OSA forms MUST be submitted a minimum of 48 hours prior to the first game the player wishes to play.
Unfortunately, we cannot accept credit card payments at this time; therefore, payment needs to be made by cash or cheque. Please print a copy of your registration and OSA form to submit with payment. Payment can be brought to Beckwith Township office, mailed to the township office, or given to the league convenors prior to the first game of the season.

PLEASE NOTE: The game sheet has been created using Microsoft Word; therefore, you are able to download, fill out, and save the game sheet.
REGISTRATION REQUIREMENTS
PLEASE NOTE: Before a player is considered eligible to play in any division, ALL forms (individual registration and OSA Insurance and Liability) MUST be submitted a minimum of 48 hours prior to the game kick-off. The OSA’s policy states that players be registered in their system 48 hours before commencement of play. This means that players are not allowed to play until their forms have been processed by the Carleton Place Soccer Club. Registration and OSA forms can be submitted at the Township of Beckwith Municipal Office, located at 1702 9th Line Beckwith. The Township of Beckwith Municipal Office is open Monday - Friday from 8:30 a.m. - 4:30 p.m. There is a mail slot in the Township Office front door for after hours drop off.
Team captains are responsible for providing the league convenors with a completed team roster, including signatures, prior to the first game of the season. Players who are newly registered to a team can be added to the team roster prior to his/her first game.
